Professional Development Hour (PDH) Certificates of Attendance are issued to all Members in good standing following a workshop or conference and as of February 2017 for Breakfast Series (certificates will not be issued retroactively - pre-2017).

Non-members may request a PDH Certificate of Attendance at a cost of $25 per Certificate.

Please complete the form to request a Certificate of Attendance for Professional Development Hours.

Request must be received within 1 month of the event.

Upon submission of the form data please proceed to our payment page (via PayPal) to confirm your request for a PDH Certificate.  You will receive a receipt (from PayPal).  Should you not receive an email please contact info@awma.on.ca.
